* Develop and implement comprehensive assessment frameworks to inform high-quality care plans for adult clients
* Collaborate closely with multidisciplinary teams to deliver holistic person-centred services/li>Maintain accurate clinical records and participate actively in case conferences meetings/li>Support colleagues in developing maintaining their skills competencies/training needs
