The purpose of this Clinical Nurse Specialist, candidate (cCNS) - Respiratory Pulmonary Rehabilitation post is to:Deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008.The successful candidate will work as a key member of the multidisciplinary Pulmonary Rehabilitation team providing a patient/client centred quality service to patients/clients and this specialist practice will encompass a major clinical focus which comprises assessment, planning, delivery and evaluation of care given to patients/clients and their families.The role of the Clinical Nurse Specialist, candidate (cCNS) - Respiratory Pulmonary Rehabilitation will differ according to the needs and configuration of established respiratory services at each site. The successful candidate will work within the Community PR service and integrate with other ambulatory care services between the hospital and community. They will work with colleagues across these services to develop and implement ambulatory care pathways to manage patients with respiratory disease and associated co-morbidities, within the community setting. The post holder will work as part of a multidisciplinary team delivering coordinated evidence based care for patients.Pulmonary Rehabilitation has been proven to increase exercise capacity and health status in people with respiratory disease who have significant self-reported exercise limitation. It can improve exercise capacity in people with a variety of respiratory diseases that affect activities of daily living.The role of the Clinical Nurse Specialist, candidate (cCNS) - Respiratory Pulmonary Rehabilitation is to be responsible for the safe and competent assessment and supervision of patients with respiratory disease undertaking PR programmes. The Clinical Nurse Specialist, candidate (cCNS) - Respiratory Pulmonary Rehabilitation is required to provide a specialist nursing resource within the governance structure created for the programme. They will provide physical, psychological and emotional support to PR participants and their families as needed throughout the programme.The post holder will have a close working relationship with their physiotherapy team members. They will also liaise closely between the PR team and other integrated respiratory care teams in the community and secondary care to deliver effective evidenced based care, using resources efficiently to achieve the best possible outcomes in keeping with the National Clinical Programmes-Respiratory guidance documents.In order to ensure continuity of service to patients the Clinical Nurse Specialist, candidate (cCNS) - Respiratory Pulmonary Rehabilitation may be required to rotate/cover other parts of the integrated respiratory service such as Respiratory Integrated Care and Outreach. This will also promote service integration and enhance skillset development. It will be dependent on identified service needs at each site. If deemed appropriate, site rotation should occur within the first 3-6 months of taking up the post. This post will be substantively based within the Chronic Disease hub/Primary Care.
