Education/Experience Requirements:
* QQI Level 5 Social, Healthcare degree preferred.
* 2-3 years’ experience in home care, healthcare, or senior-related industry required; an equivalent combination of education and work experience may be considered.
* Computer literacy skills is a must.
Responsibilities of the Care Manager:
* Conduct consultations and prepare care plans.
* Perform risk assessments in clients' homes and mitigate risks.
* Monitor and manage client and caregiver concerns.
* Conduct quality assurance visits, develop and review care plans, and coordinate with families and healthcare professionals.
* Ensure care delivery complies with standards and policies, including HCCI, ISO, and HSE requirements.
* Supervise and mentor caregivers.
* Develop and maintain relationships with clients, families, and healthcare professionals.
* Report clinical issues to senior management.
* Participate in clinical audits and other delegated duties.
