Role Summary***
The role of Support Coordinator is focused on helping clients understand and implement the funded supports in their NDIS plans, while connecting them with the right services to meet their needs. You will work closely with clients and their families or representatives to build their capacity to confidently manage these supports. Above all, this role is about going the extra mile and empowering clients to achieve their goals and pursue the life they aspire to.
*Responsibilities And Duties*
The Support Coordinator role focuses on assisting clients to understand and implement the funded supports in their NDIS plans, while connecting them with appropriate services to meet their individual needs.
*Key Duties Include:*
* Accepting and actioning referrals for NDIS Support Coordination
* Partnering with clients and their families/representatives to strengthen their ability to manage supports within their NDIS plan
* Navigating and working effectively within the NDIS funding framework and terms of business
* Responding appropriately when a client's needs change or escalate
* Building and maintaining strong relationships with internal and external stakeholders
*Skills, Attributes And Experience*
Our ideal candidate will bring experience in case management or support coordination within the Disability sector, with a strong understanding of the NDIS framework and terms of business. Tertiary qualifications in a relevant humanities field are desirable; however, what matters most is your kindness, empathy, communication skills, and ability to work independently while remaining connected to a supportive and collaborative team.
*Key Criteria Includes:*
* Proven experience in a support coordination or similar role within the NDIS framework
* Strong interpersonal skills to build and maintain meaningful professional relationships
* Creativity and resourcefulness in problem-solving
* Self-motivation and drive to consistently deliver positive outcomes for clients
* Must obtain or be willing to obtain NDIS Worker Screening Check, Police Clearance and WA Working with Children Check
