Job Title: Senior Care Coordinator
The primary goal of this role is to provide care and support through a person-centred approach which meets the practical, physical and emotional needs of service users with an Intellectual Disability/ABI/Autism.
Main Responsibilities:
* Provide care and support to residents with intellectual disabilities/ABI/Autism in collaboration with healthcare professionals.
* Assist individuals with disabilities in achieving their highest level of functioning by facilitating residential, training, and day service activities.
