TerraGlen is devoted to working in partnership with young people, their families, representatives, and others to deliver a specialist, person-centred, and outcome-focused service for individuals with disabilities.

All staff employed by TerraGlen are expected to:

* Work within the context of TerraGlen's Vision, Mission Statement, and Philosophy.
* Work to progress their professional development in line with TerraGlen proficiency model.
* Recognise that each individual is unique and work in consultation with them to enhance their strengths and further improve their quality of life.
* Adhere to TerraGlen's Code of Practice, confidentiality, code of conduct, and display a high standard of professionalism.


Responsibilities

* Motivate, support, and encourage service users to engage in planned activities of their choice.
* Support the care of the service user, including personal care and providing an overview of their daily needs.
* Support and promote independence and community integration in line with New Directions (HSE, 2012).
* Educate yourself about each service user, listen to them, and use reflective practice to develop your skills and knowledge.
* Participate in a key worker role with guidance from senior staff.
* Work as part of a team, supporting colleagues and attending regular meetings and supervisions.
* Maintain confidentiality in accordance with GDPR.
* Manage and administer medication as per policy.
* Liaise with multidisciplinary teams, families, and relevant parties.
* Observe and report on health and safety matters.
* Contribute to Person-Centred Planning meetings and follow up on actions.
* Ensure service users receive support in daily life and support their decision-making.
* Report concerns about welfare, safety, or health.
* Accurately record information following National Standards and New Directions.
* Support service users in managing behaviours of concern using positive behaviour support plans.
* Maintain high-quality service standards, adhering to audits and following up on actions.
* Follow financial procedures, reporting discrepancies.
* Build positive relationships with families and relevant individuals.
* Develop professional rapport with colleagues for effective service delivery.
* Create opportunities for community involvement, volunteering, employment, or education.
* Attend mandatory training and development programmes.
* Engage in supervision and appraisals, reflecting on practice to enhance skills.
* Perform any other duties within the scope of the social care worker role to meet service needs.


Qualifications

* Minimum Level 7 in Applied Social Care or relevant qualification.
* Full Driver's Licence with at least two years' driving experience.
* Currently undertaking or holding a CORU-recognised Social Care Qualification.
