About the Programme Facilitator Role

This role reports to the Community Service Manager through the Programmes Supervisor.

You will be responsible for facilitating service users in their choices, developing programmes within a person-centred model of service delivery.

The programmes may include independent living, personal development, and occupational and leisure activities, aligned with a quality-of-life model.


Your Key Responsibilities

1. To act as a key worker for service users, facilitating the Person-Centred Planning process.
2. To identify goals and required supports for individual service users and maintain relevant records.
3. To develop and deliver person-centred programmes to meet the needs of service users, as identified through Person Centred Planning, and maintain required records.
4. To support individuals and groups in accessing their community to meet identified needs by facilitating activities and programmes, and other reasonable duties as deemed appropriate.
5. To work independently and liaise with team members to ensure service users receive the highest standard of service.
6. To communicate effectively with external agencies and families, maintaining relevant records.
7. To adhere to all rules and regulations of the service, ensuring that administrative policies and procedures are followed at all times.
8. To promote independence and community integration, approaching interactions with service-users in a professional manner with dignity and respect.
9. To comply with health and safety legislation and maintain the highest standards of safety, hygiene, and good housekeeping within the service.
10. To ensure safe transportation of clients when using transport, adhering to safety equipment guidelines.


Requirements

* Hold a full, clean manual driving license for at least two years.
* Travel is required for this role.
* Applicants may be required to provide a police certificate if applicable.
