Organisational Context
SafetyNet’s nursing services are delivered across the Inclusion Health Hub, Jane’s Place, the Homeless Primary Care Team (Inreach & Outreach), and the Mobile Health and Screening Unit. This role primarily supports the Homeless Primary Care Team (Inreach & Outreach) service.
Job Summary
The Safetynet Homeless Primary Care Team is a multidisciplinary team consisting of GPs, 1 nurse, and 1 Key Worker, with operational oversight from the Senior Nurse Manager. The HPCT Nurse will be office‑based at 60 Amiens St, Dublin 1, providing drop‑in nursing support to people experiencing homelessness in temporary accommodations, private emergency accommodations, and through assertive street outreach on both sides of Dublin City.
The HPCT Nurse is responsible for the daily operation of the service, triaging referrals, scheduling drop‑ins and outreach assessments, and ensuring safe and efficient delivery. The role includes hands‑on nursing care in emergency accommodation and assertive street outreach, supporting GPs with follow‑up and coordination of patient care, and ensuring clinical services run smoothly, safely and in line with best practice.
As SafetyNet continues to adapt to the needs of our clients, the services requiring nursing support may develop and change over time.
Key Responsibilities

Triage and follow‑up referrals to the HPCT
Weekly scheduling and support of GP drop‑ins and outreach visits
Maintain clinical stock and equipment for HPCT services
Assertive street outreach with the Dublin Outreach Team to coordinate healthcare for rough sleepers
Advocate, coordinate, and liaise with partner health and social care providers (e.g. community GPs, Homeless Health services, mental health services, secondary/tertiary facilities, addiction services, harm‑reduction services, peer advocacy services)
Monthly meetings with the HPCT GP lead and Senior Nurse Manager to plan and coordinate the service
Support compliance with HIQA regulations and stay informed of the regulatory environment
Maintain good employee relations and promote open communication with staff and teams
Adhere to nursing practice standards, behaviour guidelines, and SafetyNet standards
Undertake other duties as determined from time to time

Clinical Responsibilities

Provide safe, comprehensive nursing care to service users following the Code of Professional Conduct and Clinical Guidelines
Practice nursing in accordance with National, HSE, and local policies, protocols, and laws
Manage own caseload in line with the post requirements
Daily management and follow‑up of blood results and appointments
Provide nursing care including, but not limited to:
Chronic Disease Management
Women’s & Sexual Health

Administrative & Health & Safety

Liaise and network with community and hospital health professionals
Assist with clinical policy and procedure development
Ensure adherence to risk management strategy and related requirements
Observe and enforce policies such as health and safety, infection control, and controlled drugs use
Develop and manage effective safety procedures in compliance with statutory obligations
Report and take action on hazards affecting staff and service users
Provide weekly service updates to Senior Nurse Manager and Services Manager
Maintain clinic settings, order supplies, and ensure equipment is clean and functional
Identify, assess, manage, and monitor risk within area of responsibility and report to senior management as needed
Complete incident, near‑miss, and clinical risk reports, promoting a “no blame” culture
Maintain accurate written and electronic nursing records and reports in line with guidelines
Implement safety procedures to meet statutory obligations per SafetyNet Health & Safety Statement

Engage in continuing professional development and stay up to date with nursing literature and emerging practices, including staff study days when appropriate
Perform administrative tasks such as word processing, filing, record keeping, database input, and report generation
Initiate or participate in research studies as appropriate
Attend internal and external meetings as required
Engage in SafetyNet supervision and mentorship with the Line Manager

Person Specification
Registered General or Psychiatric Nurse with the NMBI, minimum two years post‑registration experience (including at least one year in a relevant specialty), evidence of ongoing CPD, and clinical, administrative and professional competence to deliver high‑quality care. Demonstrates evidence‑based clinical knowledge, understanding of legislation, HIQA standards, and professional guidelines. Capable of planning, prioritising and making sound decisions in a fast‑paced, complex environment. Experience in general practice and/or working with marginalised or socially excluded groups is desirable. Excellent communication, interpersonal, IT, and independent and collaborative working skills are required. Resilience, adaptability, and leadership in clinical practice are essential, alongside treating all service users with dignity and respect. A strong interest in inclusion health, commitment to positive change, and adherence to SafetyNet values and professional conduct are expected.
Employment Details

Appointment: Whole time, permanent, pensionable
Probationary Period: Six months
Annual Salary: €37,744 – €56,587 per annum, in line with the applicable salary scale
Working Hours: 35 hours per week, Monday–Friday (start time may vary)
Annual Leave: 25 days per annum
Location: Dublin, with travel required across sites
