Family Multi-Disciplinary Team (MDT) Manager Job Description
The primary purpose of this role is to coordinate, plan and deliver a Multi-Disciplinary Team in the Dublin region to work with families experiencing homelessness or who have recently exited homelessness.
This role requires strong leadership and management skills to develop relationships and structures with relevant stakeholders. The successful candidate will also need to appraise and develop service options for vulnerable families, as required.
Key Responsibilities:
1. Develop and deliver an MDT service for families with complex needs.
2. Establish working MOUs to deliver clinical aspects of the recommended MDT service.
3. Work closely with heads of service to develop frameworks and practices for delivering a fully realised MDT service.
4. Project manage the delivery of the new MDT service and report on progress.
5. Develop a costed business case for provision of services.
6. Maintain accurate records and reports.
7. Efficiently manage budgets and resources.
8. Provide data and statistics as requested.
9. Develop interagency links with statutory, voluntary and private service providers.
10. Attend meetings and training sessions, as required.
11. Implement policies and procedures.
12. Engage in supervision sessions.
13. Undertake other duties as assigned.
Requirements:
1. A recognised third level qualification in social care/work, nursing, public health, psychology, addiction studies or a related field.
2. At least two years' management experience working with homeless people, substance users, people experiencing poor mental health and/or other socially marginalized groups.
3. Experience of working with vulnerable families with complex needs.
4. Excellent communication and interpersonal skills.
5. In-depth knowledge of key principles of care & case management approach.
6. Full Driver's License with access to a car.
