About L'Arche Cork

L'Arche Cork provides residential and day services to people with intellectual disabilities. The L'Arche model is relationship based, and at the centre of the model are volunteers who commit to the community for a period of time, normally a year, and share life with L'Arche Core Members (residents). The volunteers greatly enhance the lives of the Core Members and vice versa. The model is supported by professionally qualified staff, and each house has a house leader, a deputy and/or other care staff, based on the needs of each house. The five houses are also overseen by two Persons in Charge. The Community Nurse works closely with the care team in terms of coordinating and overseeing all elements of care and relevant training.

Essential for the Role:

* Either RNID or RGN qualifications and be registered with An Bord Altranais.

* Be interested in the L'Arche model of care

* A full clean driving license

* Be a team player

* Proactive and flexible in approach

Key Tasks

All duties are to be carried out in line with the L'Arche ethos, An Bord Altranais guidelines and HIQA standards.

In relation to Core Members:

* Coordinate assessment, care planning, implementation, and review processes in

collaboration with each core member's multi-disciplinary team ensuring alignment with HIQA regulations and best practice guidelines.

* Deliver safe, evidence-based nursing care for the assessed needs of the individual and develop agreed care plans.

* Maintain accurate documentation in compliance with professional and organisational requirements in line with GDPR principles

* Uphold principles of choice, rights, inclusion, accountability, and unconditional support.

* Support audits, incident reporting and risk management.

* Participate in policy development and service improvement initiatives.

* Promote health and wellbeing for all core members.

* Liaise with families and other agencies involved in the core members care, as appropriate and according to a person's will and preference.

* Report to Persons in Charge.

* Provide reports as required including monthly and annual report.

* Keep up to date with nursing literature and new developments in intellectual disability policies, education and practice
* Comply with mandatory training requirements

* Adhere to relevant legislation and regulations.

* Liaise with the GPs, consultants and allied professionals in relation to health issues.

* Have good protocols in place for minor illnesses, PRN use, long term conditions and the overall medical care of the residents
* Follow up on medical appointments
* Accompany core members to their GP, specialist or hospital appointments as needed and advocate for core members around their health issues.
* Establish with staff and volunteer assistants, an understanding of medical issues, and interventions.
* Assisting core members in understanding their medical concerns and medication.
* Arranging annual medical checks.

* Supporting core members around mental healthand referring them to the appropriate sources of expertise:

* G.P. Psychiatrist/Public Health Nurse for Mental Health/ Psychological Team

* Conduct risk assessments in relation to personal care, treatments and long-term care.

* Keeping up-to-date medical records for each resident which will involve obtaining reviewing and filing reports (e.g., following doctor's visits), ensuring core members' medical files are up to date and relevant people are informed of changes in health, medication, routines, etc.

* Health Promotion: Promoting health amongst the core members. This will include areas such as diet, exercise, hygiene, personal grooming, sexuality and relationships.

* Dealing with minor wounds/ illnesses, changing dressings for the core members.

In relation to Staff and Volunteers:

* Supporting volunteer assistants and employees in procedures relating to standard precautions, safe administration of medication administration, monitoring blood glucose monitoring levels, blood and body fluid spills, needle stick injuries, etc.

* Provide ongoing updates, instruction and monitoring for staff and volunteers.
