Job Description
Liaise with families, schools and day programme staff to ensure continuity of support. Ensure delivery of appropriate person centred care and support for daily living activities. Provide leadership supervision and performance management for assigned staff.
Key Responsibilities
* Lead and support staff in delivering high-quality services for individuals with intellectual disabilities.
* Oversee implementation of each person's Personal Plan through regular reviews.
The role requires a minimum of a Level 7 degree in Social Care or equivalent.
