Clinical Nurse Specialist – Acute Stroke and Endovascular Thrombectomy Service

Principal Duties And Responsibilities
Practice nursing in line with the Code of Professional Conduct, Scope of Practice Framework, Nursing Board of Ireland, and National Clinical Guidelines for Stroke.
Work within scope of practice and develop necessary competence.
Maintain high standard of professional behaviour; ensure patient and family care aligns with mission, vision, values, and strategic plan.
Apply ethical policies and procedures pertaining to the area of practice.
Follow appropriate lines of authority within Nurse Management structure.
Hold a recognised Irish Driving Licence.
Comply with all existing Hospital policies.
Responsibilities
Primary Role – Thrombectomy Service
Develop stroke services in line with Stroke Care National Clinical Programme, guidelines, and thrombectomy service reference.
Collaborate with multidisciplinary team to assess, plan, implement, evaluate care plans for stroke patients.
Conduct nursing assessments (Barthel Index, MMSE, NIHSS, GCS, mRS, etc.).
Plan individualised care aligned with MDT and protocol-driven guidelines.
Evaluate care through audit and research; use outcomes to inform service and patient family communication.
Make variations in clinical options within agreed protocols.
Build links with other stroke services; participate in stroke networks.
Promote liaison & coordination of care for patients between services and hospitals.
Lead development of information sharing protocols, audit systems, referral pathways, care plans, shared arrangements.
Identify professional and voluntary resources at local and national level for referrals.
Engage in departmental Clinical Governance processes with Head of Department, Assistant Director of Nursing, CNM3, Clinical Governance lead, and stroke governance group.
Maintain effective clinical governance procedures and evolve them as needed.
Secondary Role – Wider Stroke Service
Provide efficient, high-quality service respecting patient needs (e.g., same-day tPA assessment, history, acute complication management, education for patient and family).
Manage caseload: same-day referral assessment, weekly inpatient education, OPD secondary prevention, MDT meetings, staff education.
Accept appropriate referrals from MDT and allied health professionals.
Collaborate with consultant staff to refer patients for diagnostics per protocols.
Patient Advocate Role
Advocate for patient values and decisions; collaborate with MDT.
Liaise with health services; coordinate weekly MDT meetings.
Develop advocacy concepts for patient participation in decisions, enabling informed choice via education.
Maintain and improve collaboration procedures between acute services.
Take action on matters detrimental to staff or service user care.
Educational Role
Maintain clinical, nursing, managerial competence; keep up to date with research for evidence-based practice.
Attend seminars, read literature, update knowledge of nursing policy, standards.
Identify educational needs of patients, carers, professionals, students; participate in training programmes.
Supervise student nurses; support their personal and professional development.
Provide learning opportunities to all staff; collaborate with Nurse Education Centre.
Participate in orientation programs for new staff and in-service programmes.
Consultant Role
Provide specialist knowledge, expertise and care in liaison with MDT.
Promote Stroke Care Nursing Service role among staff.
Support, advise, and help build expertise of other professionals through formal and informal education.
Offer resource support to hospitals caring for post-thrombectomy patients.
Ensure guidelines for National Emergency Thrombectomy Service are developed, agreed, and circulated with MDT.
Audit and Research
Manage Stroke Register information; maintain monthly statistics (referrals, patient numbers, tPA figures, LOS, outcomes).
Collect national thrombectomy data for audit; follow-up phone calls to record outcomes.
Promote evidence-based interventions; lead organisational audit to evaluate service effectiveness.
Engage in nursing research; present findings to MDT.
Act on audit findings; implement changes; develop KPIs.
Health & Safety – Management
Follow Irish Healthcare System policies, procedures, legislation.
Document near misses, hazards, accidents; report to relevant person.
Adhere to risk management; ensure safe practice.
Follow department procedures for assistive devices.
Promote diversity, respect, and HIQA Standards.
Administration
Process complaints per Beaumont policy.
Contribute to policy development, performance monitoring, budgeting.
Safeguard records per local policy and legislation.
Maintain accurate records and data for service planning, delivery, evaluation.
Support IT developments.
Undertake additional duties as directed by Director of Nursing.
Qualifications
Selection Criteria
Statutory registration as RN/Midwife on active register of Nurses and Midwives.
Registered in relevant division(s) of the Nursing and Midwifery Board of Ireland.
Minimum 1 year post-registration full-time experience (or aggregate) in stroke care.
Successful completion of post-registration study: Level 9 or higher; equivalent to 60 ECTS.
Continuing professional development relevant to specialist area.
Demonstrate safe and effective practice within scope.
Ability to practice safely in managerial and administrative roles.
Maintain live annual registration and PSAC confirmation.
Good character and competency in health and leadership capacities.
Abatement provisions apply to civil/public service pensions (if applicable).
