About the Role:
The post holder will facilitate service users in their choices, developing programs within a person-centred model of service delivery.
* The programs may include independent living, personal development, and occupational and leisure activities, within the context of a quality-of-life model.
Responsibilities:
1. To be a key worker to service users, facilitating the Person Centred Planning process.
2. To identify goals and supports required by individual service users and maintain records as required.
3. To develop and deliver person centred programs to meet the needs of service users, as identified through Person Centred Planning, and maintain the required records.
4. To support individuals and groups in accessing their community to meet identified needs, by facilitating activities and programs, and any other reasonable duties deemed appropriate by the Programs Supervisor/Community Services Manager.
5. To work independently and liaise with relevant team members to ensure service users receive the highest standard of service.
6. To communicate with external agencies and families, and maintain the relevant records.
7. To observe all rules and regulations of RehabCare services, ensuring all relevant Rehab Group administrative rules, policies, and procedures are adhered to at all times.
8. To promote and support independence and community integration in your approach with the service user group, including liaison with key community-based personnel, in pursuit of accessible community activities, in accordance with the service user's choices and aspirations.
9. To comply with health and safety legislation and maintain the highest standards of safety, hygiene, and good housekeeping within the service.
10. To ensure the safe transportation of clients when using transport, by adhering to the use of all safety equipment, as appropriate.
11. To ensure all interaction with the service-user is approached in a professional manner, with dignity and respect, and report any concerns to the Programs Supervisor or Community Services Manager as appropriate.
Additional Requirements:
* A full driving license is essential for this role, which requires travel.
* A minimum QQI Level 5 in Social Care or equivalent qualification is required.
* A third-level graduate, Health & Social Care, or equivalent, is desirable.
* Programme facilitation skills are an advantage.
* Client handling experience is an advantage.
