The Role
The Addiction Counselor will manage a caseload of patients, providing individual and group counseling, conducting assessments, coordinating services, psycho-education, referrals, and ensuring all documentation is up-to-date.
Responsibilities
* Develop treatment plans in collaboration with the Program Director or Clinical Supervisor, tailored to patients' physical, medical, cognitive, and psychosocial needs.
* Utilize a strengths-based and trauma-informed approach to support patients' recovery, working collaboratively with patients and treatment team members.
* Provide clinical consultation to referral sources, mentors, family members, and others to support patient recovery.
* Engage with patients to involve them in decision-making and help them understand assessment benefits and appropriate care levels.
* Review referral information and coordinate with referral sources to support service delivery and schedule patient interviews.
* Make referrals based on assessments to ensure patients access appropriate programs and services.
* Assist patients with scheduling admissions or initial appointments at referred programs or facilities.
* Monitor patient engagement with treatment and address barriers to recovery.
Qualifications
We are seeking a highly motivated and experienced professional with:
* A High School Diploma or Equivalent
* At least one (1) year of experience working with clients with Substance Use Disorders (SUD)
* Knowledge and/or training in Trauma-Informed Care and organizational treatment approaches
* Experience working with individuals with co-occurring disorders
* Understanding of laws regarding personnel and client confidentiality
