About the Role
To promote and support independent living and community integration, facilitating individual growth and development through various programmes and activities.
Key Responsibilities:
* A commitment to organisational values of Teamwork, Dignity, Justice, Advocacy and Quality is essential.
* You will be a key worker, supporting service users in their Person Centred Planning process, identifying goals and required supports.
* Maintain accurate records of service user progress and programme delivery.
* Develop and deliver person centred programmes tailored to meet individual needs, as identified through Person Centred Planning.
* Conduct daily duties such as health and safety checks, administrative tasks, and supervisory responsibilities, ensuring effective programme management.
This fixed-term contract offers a 12-month opportunity for variable hours per week.
