The Role
* Maintain patient/resident confidentiality at all times.
* Assist residents with all activities of daily living.
* Conduct assessments, plan, implement, and evaluate social work programs for residents according to professional standards.
* Manage patient care to ensure the highest professional standards, utilizing a personalized approach.
* Communicate and coordinate with residents, staff, and family members to provide the best possible care.
* Participate actively in multidisciplinary healthcare teams and work collaboratively as part of the team.
* Keep accurate records of assessments, interventions, and communications related to residents.
Required Degree
* A relevant qualification at Level 7 or higher in Social Care or hold a CORU registration under the Social Care Worker division.
* Bachelor of Arts in Social Studies.
* Bachelor of Arts in Applied Social Studies in Social Care.
* Bachelor of Arts in Applied Social Care.
* Bachelor of Arts (Honours) in Social Care.
