Responsibilities:
* Conduct comprehensive psychosocial assessments and develop individualized care plans for older adults.
* Provide therapeutic interventions and support to patients and their families.
* Collaborate with multidisciplinary teams to deliver holistic care.
* Advocate for patients' rights and access to services.
* Maintain accurate and up-to-date case records and documentation.
* Participate in team meetings, case conferences, and training sessions.
Requirements:
* Minimum of a level 8 degree in Social Care.
* 3 years post qualifying experience to work at Senior Social Worker level 8.
* CORU registration.
* Experience in mental health services, preferably with older adults.
* Strong assessment, intervention, and case management skills.
* Full driving license and access to own car is desirable.
* Excellent communication and interpersonal skills.
* Resident in Ireland and legal right to work in Ireland without restriction.
* All candidates will be required to undergo Garda Vetting.
* International Police Clearance (if lived outside of Ireland for more than 6 months after the age of 16).
