Job Description
The Support Programme Coordinator is a key role within our organisation, responsible for meeting the practical, physical and emotional needs of individuals in our care.
We are seeking an experienced professional to design individual support programmes, coordinate their delivery, and ensure that work is tailored to meet the unique needs and preferences of each individual. This will involve working closely with families and multi-disciplinary teams to promote the best possible outcomes for those we support.
* Plan, coordinate and implement safe transitions into our service and community, taking into account the individual's physical, emotional and psychological needs.
* Liaise with family members and other stakeholders to gather information and provide updates on progress.
* Develop and implement individualised programmes to enhance capabilities and independence, promoting personal growth and development.
* Provide support and guidance to individuals as needed, ensuring that their physical, emotional and personal needs are met.
* Ensure that medication is managed safely and effectively, in accordance with our organisation's policies and procedures.
