Overview of the Role
Riverstick Care Centre is a facility that provides long term care and short‑term care respite/convalescence. We also have a Transitional Care Unit (TCU) at Riverstick Care Centre that provides a vital bridge between acute hospital care and a patient’s return to home or long‑term care setting. Designed to support individuals who no longer require acute hospital treatment but are not yet ready for discharge home, the unit delivers high‑quality, person‑centred care focused on recovery, functional independence, and safe transition. A core function of the Transitional Care Unit is the effective management of admissions, discharges, and patient flow. Referrals are reviewed through a Trusted Assessment process, in collaboration with senior nursing leadership, to ensure each admission meets defined inclusion and exclusion criteria and that the unit is the most appropriate setting for the patient’s needs. This structured approach supports safe placement, efficient use of resources, and timely transfers from acute hospitals.
The CNM will work as part of the centre management team, to ensure the safe and effective management of patient referrals.
The CNM will carry out a range of duties to ensure that we provide a quality, person centred care environment for all residents. The CNM will work on the floor and assist in coordination of admissions and discharges to the TCU. The CNM will demonstrate managerial and leadership skills and facilitate effective communication with colleagues in the nursing home.
Roles & Responsibilities

Oversee and coordinate all admission and discharge processes to ensure timely, safe, and efficient patient transfers to Riverstick Transitional Care Unit.
Receive patient referrals/trusted assessment from the key referral sources i.e. Cork University Hospital and liaise with the referral teams / Discharge Coordinators.
Optimise patient flow by managing bed utilisation, length of stay, and throughput in collaboration with CUH discharge Co Ordinator's Bed management teams and Clinicians.
Ensure smooth transitions of care between acute care, TCU, and post‑discharge settings.
Address barriers to patient flow and implement strategies to improve efficiency and continuity of care.
Confirmation of the Trusted Assessment in collaboration with the Director of Nursing (DON) and Assistant Directors of Nursing (ADONs), ensuring the referral meets established inclusion and exclusion criteria and is appropriate for placement at Riverstick Care Centre.
Arrange for accepted referrals to be transferred to Transitional Care Unit.
Working to support the coordination of transfer to the TCU to ensure patient flow in the centre as required.
Ensure all admissions to the Centre are carried out correctly and all appropriate documentation completed.
Attend daily handovers and weekly MDT meetings.
Ensure data collection for weekly report submission to CUH i.e. DTOC list, bed allocation KPI related data.
Prepare patient listing, coordinate the MDT and record MDT minutes.
Compile discharge documentation and forward to GP’s PHN and community network team.
Maintain patient data board to maintain patient flow.
Deliver clinical support across units to ensure continuity of care.
Liaise with TCU nursing team about admissions, prescriptions and ensure admission paperwork is prepared and available.
Follow‑up on patient's outpatient appointments if necessary.
Facilitate patient admissions/discharges.
Facilitate effective communication with patients, relatives, visitors, and multi‑disciplinary teams as necessary and ensure patient confidentiality is respected and maintained at all times.
Ensure that the residents’ register is kept updated for all admissions and transfers.
Lead family meetings to ensure that residents’ needs are met in this regard.
Adhere to Silver Stream guidelines, policies, and procedures.
Maintain a safe working environment as set out in the Health and Safety at Work Act.
Respect confidential information obtained in the course of professional practice.
Work within the code of Professional Conduct for Nurses, participation in the development of the Scope of Practice and provide strong professional leadership and direction for nursing services.
Work in accordance with HIQA guidelines and standards. To have the knowledge and understanding on implementing the guidelines when a HIQA inspection is carried out.
Promote the advancement of professional knowledge and ensure its integration into practice.
Be familiar with Health and Safety Regulations as they relate to the Home.

Skills and Experience Required

3 years post‑registration experience in a relevant field and experience in managing patient caseload.
Hold a recognised Managerial Qualification (minimum level 6 on the National Framework of Qualifications or be working towards this level of qualification).
Registered on the NMBI live register.
Clinical experience working in older persons care or nursing home care.
Working experience of HIQA Standards and Health Act Regulations 2013.
Fluent in English language (verbal and written) and be eligible to work in Ireland.
Strong communication and listening skills.
Ability to work well as part of a care team.
Caring and compassionate and be passionate about providing high quality person‑centred care.
Have a good knowledge of IT systems such as excel spreadsheets.
Have a good understanding of applications for home supports and NHSS/Fair deal.
