About This Role
* Conduct thorough assessments of clients' medical conditions, functional abilities, and psychosocial needs.
* Develop tailored care plans based on assessment findings.
* Collaborate with healthcare professionals to ensure a multidisciplinary approach to client care.
* Coordinate with community support services to arrange necessary healthcare resources.
Your Qualifications
* Bachelor's degree in nursing or a related healthcare field.
* Valid nursing license or relevant professional certification.
* Minimum 2 years of clinical experience in a hospital or community healthcare setting.
