Job Title: Disability Support Specialist
We are seeking an experienced and compassionate individual to join our team as a Disability Support Specialist. In this role, you will play a crucial part in promoting independent living and community integration for individuals with disabilities.
The successful candidate will have a strong background in social care, with experience in facilitating person-centered plans, designing and delivering programs and activities that meet the unique needs and abilities of service users. You will be responsible for working one-on-one with clients, providing support with daily living skills and community integration, while also maintaining a focus on health, safety, and hygiene.

As a Disability Support Specialist, you will be required to:
* Develop and implement personalized plans to promote independence and community integration
* Provide support with daily living skills, such as personal care, meal preparation, and household management
* Facilitate community-based activities and outings to promote socialization and participation
* Maintain accurate records and reports, ensuring compliance with relevant regulations and standards
* Collaborate with other professionals, including healthcare providers and family members, to ensure seamless support and coordination

Requirements:
* Minimum QQI Level 5 in Social Care or equivalent qualification
* At least 1 year of experience in a relevant setting, preferably in a disability support role
* Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues
* Ability to work independently and as part of a team, with a high level of flexibility and adaptability
* Full driving license, with access to reliable transportation
