NHS Fife Internal Only Applicants - you must be currently employed by NHS Fife to be eligible to apply for this post****An opportunity has arisen within our Hospital at Home service for a highly motivated Nurse Practitioner to join our East Team based at Adamson Hospital. This role will support the H@H team to deliver safe, effective and person centred care to patients within the primary care setting. The role will predominantly focus on the Urgent and Unscheduled Care needs of the older adult population and the complex treatment and management of acutely unwell, palliative or frail patients including: nursing and care home residents.It is recognised that as people live longer, services and the way we work together has to change if we are to continue to meet the needs and aspirations of our communities. In Fife Health and Social Care Partnership there is a recognition that having staff with the right knowledge and skills within the community is required to support patients to live long and healthy lives in their own home or in a homely environment.Nurse Practitioners must hold Clinical assessment and Decision Making module and preferably have also completed (Level 11) Non-Medical prescribing. Trainee Nurse Practitioner applicants will be on an annex 21 until they have completed clinical decision making and the competency framework*.You should have excellent communication skills and be able to work as a member of the wider Health and Social Care Team with the ability to work autonomously utilising your own initiative, clinical judgement and decision making. As this is a community post you will also be required to travel across Fife in a timely manner to attend to acutely unwell patients.This posts involves 7 day working covering 08:00 – 22:00 and is based within the Adamson Hospital.annex 21 - Candidates who commence on band 5 will be required to successfully complete the appropriate qualification/s or training as outlined within the Annex 21 competency framework prior to progressing to band 6.
