Support Worker (Outreach Service)
Permanent - 35 hours per week
Supervisor/Instructor Salary Scale €35,432 to €51,373
The Role:
The Support Worker, along with the Outreach Team will ensure individuals are respected, treated with dignity, supported to make personal choices and decisions on a daily basis.
Responsibilities:
Take part in the completion of the Assessment of Need for individuals
Support the individual to enhance their life by playing a role in ensuring that the goals and objectives are achieved in accordance with their Support Plan
Support each individual where appropriate with regards to their health
Support each person where appropriate with regards to their personal and intimate care
Maintain effective communication with families and relatives, where appropriate.
Support the Individual to make and maintain good relationships
Maintain high professional standards and confidentiality
Requirements:
Minimum Qualification: QQI Level 5 Intellectual Disability Practice - 5 Components - must include Empowering the Individual, Intellectual Disability and Community Inclusion (Essential)
Hold a full, clean driving licence and access to own vehicle (Essential)
1 years' experience working in a Local(Day) Service/Community House
Be familiar with policies and procedures in relation to the Safeguarding of Vulnerable Adults
A proven track record in implementing an Individual Support Plan
Experience administering medication and supporting individuals with personal care
Enjoy working on your own with a person with an intellectual disability
Ability to build trusting and respectful relationships
Use your own initiative to get the job done and complete household tasks
IT skills and the ability to carry out administrative tasks
Be flexible in your working arrangements
