Clinical Nurse Specialist Role Summary
* As a Clinical Nurse Specialist, you will be responsible for developing stroke services in line with the Stroke Care National Clinical Programme Model of Care.
This role involves nursing assessment and care planning, working closely alongside multidisciplinary teams to develop individualized patient plans. You will evaluate quality of patient care through audit and make variations to prescribed clinical options within agreed protocols.
Key Responsibilities:
* Nursing Assessment: Using tools such as Barthel Index, Cognition-MMSE, neurological status-NIHSS/GCS, premorbid status-m RS etc.
* Planning Patient Care: Developing individualized plans based on multidisciplinary team discussions and agreed protocol-driven guidelines
