Job Title: Health Inclusion Worker, Housing First, Limerick

Job Overview:
The primary purpose of this role is to work as part of the Focus Ireland Housing First Service to deliver a ‘Housing First’ approach to individuals experiencing homelessness across Limerick. The aim is to develop and improve wrap-around health supports in ‘Housing First’ tenancies and address complex health needs related to General & Mental Health and Addiction.
Key Responsibilities:
1. Work within the framework of the overall objectives of the Housing First Service.
2. Provide a safe, welcoming environment for customers, including basic physical care and support, respecting consumer choice and self-determination.
3. Establish trusting relationships with customers, especially those living on the streets or in temporary accommodation, to help them move towards a more stable lifestyle.
4. Undertake assessments and support planning using the Care and Case management model.
5. Work a flexible rota as agreed with the Housing First Health Co-ordinator.
6. Support customers on appointments to other services (e.g., doctors, counsellors).
7. Work as part of a multidisciplinary team and service.
8. Provide advice, information, and guidance to prevent homelessness or respond to being out of home.
9. Assist customers in accessing clinical, medical, and other support services.
10. Support customers in developing life skills, education, training, and employment opportunities.
11. Maintain customer caseloads, records, logs, assessments, and input data into the Focus Ireland database and PASS.
12. Collaborate with other teams within Focus Ireland and external agencies.
13. Attend meetings, training, and supervision sessions as required.
14. Supervise students or volunteers as assigned.
15. Implement policies and procedures, and contribute to their development.
16. Plan, implement, and evaluate daily activities.
17. Ensure health, safety, and welfare in the workplace.
18. Monitor and report on the progress of health and support initiatives.
Requirements:
* A recognised third-level qualification in social care (minimum Level 8), mental health nursing, psychology, addiction studies, or related field.
* At least two years’ experience working with homeless individuals, substance users, or those with complex health needs.
* Experience working with at-risk customers and liaising with agencies.
* Full driver’s license and access to a car.
* Excellent communication, interpersonal, and report-writing skills.
* Knowledge of Housing First principles and issues affecting individuals with complex health and addiction needs.
