Working closely with the multidisciplinary team, you will lead out on case management and care coordination, providing patient-centred, high-quality support to patients and their loved ones as they navigate the health system. The Role Work as a Medical Social Worker on an acute medical ward, providing patient-centred, high-quality social work interventions. Lead out on case management and care coordination in collaboration with MDT colleagues. Support patients and families to navigate the hospital system and the transition back to the community. Assess and address psychological, emotional, social and practical barriers to discharge. Provide timely signposting and referrals to relevant community-based supports and services. Engage in multi-agency collaboration to support safe discharge planning and continuity of care. Manage a fluctuating caseload while maintaining a high standard of care and accurate documentation. Contribute to quality improvement initiatives and practice development within the service. Key Requirements Recognised qualification in Social Work. CORU registration as a Social Worker (essential). Strong knowledge of safeguarding, capacity legislation, and ethical practice Experience working in a medical, healthcare, or hospital setting. Excellent communication, organisation and documentation skills, with the ability to manage a fluctuating caseload. A clear commitment to patient-centred care, safe discharge planning and multi-agency collaboration. Previous experience in an acute hospital/medical setting. Experience supporting complex discharge planning and liaising with community services. Experience working with adults and families experiencing illness, disability, or social vulnerability. Excellent communication, interpersonal, and multidisciplinary working skills. Ability to manage a varied and complex caseload effectively.
