The Medical Oncology Service provides general oncology care to the local population, in addition to specialised neuro-oncology care on a regional and national level. The service continues to adapt to the growing population of Ireland, the associated increase in people living with a cancer diagnosis, in addition to the growing expectations of the population in relation to their oncology care needs. The AHOS nursing service is pivotal in providing a non-ED direct access route for cancer patients who require review. Cancer patients who are on treatment and become unwell can access the AHOS nursing service via a dedicated telephone line in any 26 centres across the country that deliver SACT. The service gives patients access to specialist cancer nurse advice and support during the acute phase of their SACT treatment. Unwell patients who require additional clinical review are, where possible, seen in the cancer day care unit or a dedicated space away from the ED. This reduces the requirement for patients to attend the ED and prioritises appropriate ED referrals. The service is well placed to link with the MDT and community services as outlined in the Sláintecare strategy.ResponsibilitiesPrinciple Duties and ResponsibilitiesThe post holder’s practice is based on developing a higher level of capability across the six domains of competences as defined by Bord Altranais agus Cnáimhseachais na hÉireann Advanced Practice (NMBI 2017).The domains of competence (NMBI, 2017) are:Professional Values and ConductClinical-Decision MakingKnowledge and Cognitive CompetencesCommunication and Interpersonal CompetencesManagement and Team CompetencesLeadership and Professional Scholarship CompetencesDomain 1: Professional Values and ConductStandard 1The cANP/RANP (AHOS) will gain increased competence in applying ethically sound solutions to complex issues related to individuals and populations by:Demonstrating accountability and responsibility for professional practice as a lead healthcare professional in the care of patients within the AHOS nursing service.Collaborating with the clinical supervisor and the local stakeholder group to establish the patient caseload and scope of practice for the cANP/RANP (AHOS) nursing.Collaborating with the clinical supervisor and the local stakeholder group to determine the inclusion criteria for the advanced practice role.Collaborating with the clinical supervisor and local stakeholder group to determine the exclusion criteria for the RANP (AHOS).Articulating safe boundaries and engaging in timely referral and collaboration for those areas outside the scope of practice, experience, and competence by establishing referral pathways and locally agreed policies, procedures, protocols and guidelines to support the AHOS nursing service.Demonstrating leadership by practising compassionately to facilitate, optimise, promote and support the health and wellbeing of patients.Selecting a professional nursing practice model focusing on person-centred holistic care and therapeutic relationships.Articulating and promoting the AHOS advanced practice nursing service in clinical, political and professional contexts and contributing to service metrics and national nursing strategy as applicable.Domain 2: Clinical-Decision Making CompetencesStandard 2The cANP/RANP AHOS will enhance advanced knowledge and senior clinical decision-making by increasing competence to:Conduct a comprehensive holistic health assessment to determine diagnoses and inform autonomous advanced nursing careSynthesise and interpret assessment information to identify health statesDemonstrate timely use of diagnostic investigations to inform clinical decision makingDemonstrate knowledge of therapeutic interventions and relevant legislation and guidelinesInitiate and implement health promotion activities and self-management plansDischarge patients from the service as per policy and guidelinesDomain 3: Knowledge and Cognitive CompetencesStandard 3The cANP/RANP (AHOS) nursing will contribute to the professional body of knowledge by:Providing leadership in translating new knowledge to clinical practiceProviding ongoing education to the teamDemonstrating a vision for advanced nursing practice based on service needDemonstrating accountability in considering access, cost and clinical effectivenessDomain 4: Communication and Interpersonal CompetencesStandard 4The cANP/RANP AHOS will negotiate and advocate to ensure patient beliefs and wishes are respected by:Communicating effectively with the healthcare teamDemonstrating leadership in professional practice and documentationFacilitating clinical supervision as relevantUtilising information technology/Telehealth to record careDomain 5: Management and Team CompetencesStandard 5The cANP/RANP AHOS will manage risk by promoting a safe environment and quality care, and seek feedback from patients, families and staff.Promote a culture of quality careSeek feedback and implement practice changesCollaborate with the multidisciplinary teamDomain 6: Leadership and Professional Scholarship CompetencesStandard 6The cANP/RANP AHOS will lead in multidisciplinary planning for transitions of care by:Demonstrating clinical leadership in design and evaluation of servicesEngaging in health policy development and evaluationIdentifying gaps and leading changeAdvanced Practice Performance ManagementPerformance Indicators and national AHOS nursing metrics guide evaluation.
