This position requires close collaboration with a multidisciplinary team to ensure seamless care transitions and coordination.
Key responsibilities include:
* Providing high-quality, family-centered care to children with life-limiting conditions
* Developing and implementing individualized care plans to meet the unique needs of each child

Requirements:
To be successful in this role, you will require:
* Current registration as a Registered General Nurse or Midwife with the relevant governing body
* Minimum 3 years' experience working in pediatric nursing, preferably in a specialist setting
* Strong communication and interpersonal skills, with the ability to work effectively with families, colleagues, and other healthcare professionals
* Excellent clinical knowledge and skills, with a commitment to ongoing professional development
* Ability to work independently and as part of a multidisciplinary team
