About the Role
As a Programme Facilitator, you will play a key role in supporting individuals with disabilities and their families. Reporting to the Community Service Manager through the Programmes Supervisor, you will facilitate service users in their choices, developing programmes within the context of a person-centred model of service delivery.
The programmes may include independent living, personal development, and occupational and leisure activities, all within the context of a quality-of-life model. Your expertise will be crucial in designing, developing, and providing programmes and activities tailored to meet the needs, choices, and abilities of our service users.
We are looking for an individual with experience of working in a relevant setting, facilitating and implementing person-centred plans, and providing support in areas such as independent living skills and community integration.
You should have a strong understanding of the needs and challenges faced by individuals with intellectual disabilities, autism, mental health difficulties, and physical disabilities. Additionally, experience of lone working, supporting people in the community, and promoting self-advocacy is essential.
Responsibilities:
* To be a key worker to service users, facilitating the Person Centred Planning process.
* To identify goals and supports required by individual service users and maintain records as required.
* To develop and deliver person-centred programmes to meet the needs of service users, as identified through Person Centred Planning.
* To support individuals and groups in accessing their community to meet identified needs, facilitating activities and programmes, and any other reasonable duties deemed appropriate by the Programmes Supervisor/Community Services Manager.
* To work on own initiative as a lone worker and liaise with relevant team members to ensure that service users receive the highest standard of service.
* To communicate with external agencies and families, maintaining relevant records.
* To observe all rules and regulations of the RehabCare services, ensuring adherence to all relevant Rehab Group administrative rules, regulations, policies, and procedures at all times.
Requirements:
* A minimum QQI Level 5 in Social Care or equivalent qualification.
* A full driving licence is essential, as this role requires travel.
* Experience of supporting adults with Autism and Mental Health difficulties is highly desirable.
* Programme facilitation skills and client handling experience are also advantageous.

* Salary starting at €30,915.76 (pro rata), €15.24 per hour up to €32,518.28 (pro rata), €16.03 per hour depending on experience and qualifications.
