Key Responsibilities:

- Work collaboratively with multidisciplinary teams to develop and implement individualised plans that promote independence and community integration.
- Ensure that residents' personal plans are completed in line with regulations and standards provided.
- Participate in ongoing professional development to maintain up-to-date knowledge of social care policies and sectoral changes.

Requirements:

- Hold a qualification at Level 7 or above in a relevant area, such as Social Care, Psychology, Education, or Youth & Community.
- Possess excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and other professionals.
- Demonstrate a commitment to delivering high-quality care and support, with a focus on promoting independence and community integration.
