Job Title: Community Development Specialist

About the Role
* To facilitate service users in making informed choices, developing programs that cater to their unique needs within a person-centered model of service delivery.
Key Responsibilities
1. To act as a key worker for service users, facilitating the Person-Centred Planning process by identifying goals and required supports.
2. To design and deliver personalized programs tailored to meet the needs of service users as identified through Person Centred Planning.
3. To support individuals and groups in accessing their community by facilitating activities, ensuring accessibility and promoting independence.
Requirements
The ideal candidate will possess a minimum QQI Level 5 in Social Care or equivalent qualification, along with relevant experience supporting adults with intellectual disabilities. A third-level graduate in Health & Social Care or equivalent is preferred. Experience working independently, handling clients effectively, facilitating group activities would be beneficial candidates for this role.
