Client Care Co-ordinator - Home Support Services The person between the plan and the reality. A care plan only works if someone makes sure it actually happens - that the right carer arrives, the client is supported, the communication flows, and the small things don't fall through the cracks. That's this role. As Client Care Co-ordinator, you'll be the connective tissue between our clients, their families, our carers, and our care management team. It's part coordination, part supervision, part relationship management - and all of it matters. The details Office base: Blanchardstown, Dublin 15 Service area: Dublin North - with regular travel to client homes in your assigned region Contract: Full-time, permanent - 40 hours per week Salary: From €36,000 depending on experience and qualifications. Paid fortnightly. Reports to: Care Manager Hours: Monday to Friday, 9am-5pm, with participation in the on-call rota (additional allowance for weekend on-call, carried out remotely) Transport: Full driving licence and access to a car is essential for this role - generous travel allowance with parking and tolls fully paid What the role involves Coordinating and monitoring the day-to-day delivery of home care services for your assigned clients Liaising with clients, families, and carers to ensure care is being delivered to standard Supporting the implementation and review of individual care plans Conducting regular field visits, check-ins, and quality assurance reviews Responding to client queries and concerns - promptly, professionally, and with genuine care Providing guidance and support to carers, particularly during induction and early assignments Maintaining accurate client and carer records in OneTouch, in compliance with GDPR Flagging safeguarding concerns, incidents, and changes in client condition to the Care Manager Participating in the on-call rota as required This role requires you to deliver care directly to clients and you must be fit to work in this capacity with the relevant qualifications. Who we're looking for You've worked in care. You know what good looks like - and what it doesn't. You're the person who notices when something's off with a client, when a carer needs support, or when a family needs reassurance. You're organised enough to manage multiple clients and carers across a region, but human enough to remember that every name on your list is a person. You communicate clearly, you follow through on what you say, and you hold yourself to high standards without needing to be managed into them. You understand that quality in home care isn't abstract - it's whether Mrs. Murphy got her lunch, whether the carer knew about her medication change, and whether someone followed up when the family called. What you'll need You must have current eligibility to work in Ireland - this is a non-negotiable requirement QQI Level 5 in Healthcare - Care Skills and Care of the Older Person (minimum) QQI Level 6 in Supervisory Management or equivalent recognised qualification Proven experience in a similar coordination or senior care role in healthcare Strong communication, interpersonal, and organisational skills Proficiency with Microsoft 365 and familiarity with OneTouch Health platform Full driving licence and access to a car Ability to work under pressure in a fast-paced environment Why Hibernia Salary from €36,000 to €40,000, depending on experience.
