Role Overview
Lead the Social Care Team at Haven House, ensuring effective case management, compliance with policies, and efficient service delivery.
Responsibilities
Ensure team members implement the case management system from referral to disengagement.
Respond to child protection concerns and support families and teams.
Coordinate with Community Welfare Services for grants and settlement support.
Maintain comprehensive knowledge of local facilities, support groups, and liaise with external providers including HSE, DCC, CPS, DOSP, DRHE and other stakeholders.
Advocate for residents to secure eligible rental and social welfare supports.
Maintain detailed records of outcomes, resettlement numbers, and service statistics.
Ensure efficient and safe service operation compliant with Health & Safety regulations.
Communicate daily handover and report on service users to relevant parties.
Maintain accurate timekeeping, signing off team timesheets, and following up on absences.
Keep client notes, HNA's, Care Plans, and files in line with Crosscare policy.
Update PASS system and statistics daily.
Assist with staff rostering and accommodation of planned/unplanned absences.
Review policies & procedures and help develop new ones for safe operation.
Work an on-call system and support the management team as required.
Participate in recruitment and disciplinary, grievance processes.
Take initiative, demonstrate accountability, and act as an effective role model.
Play a floor cover role for residential service delivery.
Lead change processes, identify and promote best practice.
Perform any reasonable duties required by the Project and Deputy Leader.
Qualifications
Minimum 3 years' experience in social care, including 2 years of full working knowledge of care & case management protocols.
Minimum BA in Applied Social Care, Addiction, Mental Health, Nursing, Psychology or equivalent (Level 7 Degree).
Knowledge and experience in Child Protection.
Practical experience/knowledge of supervising staff.
Knowledge of the Pathways Model and reconfiguration of Homeless Services across Dublin.
