Clinical Pharmacist
Overview of Role:
Provide effective and efficient pharmaceutical care to all patients in our clinical setting under the direction of a Chief Pharmacist. Identify individual patient care needs and address them appropriately. Advise other healthcare professionals on patient care.
Primary Duties and Responsibilities:
1. Ensure patient care is prioritized above all else. Model UPMC's core values: Quality and Safety, Dignity and Respect, Caring and Listening, Responsibility and Integrity, Excellence and Innovation.
2. Be conversant with best practice and relevant competency frameworks for the delivery of clinical pharmacy services. Support Pharmacy Technicians in managing medicine shortages within UPMC. Participate in SOP development and implementation.
3. Contribute to medicines information provision and advice on drug availability and procurement to clinical staff. Develop medicines resources for colleagues and patients, including patient information leaflets, monographs, policies, procedures, protocols, and guidelines.
4. Collaborate in developing clinical guidelines as part of the Multi-Disciplinary Team (MDT). Identify medication management system deficiencies and address them accordingly. Highlight discrepancies, incidents, or deficits in Pharmacy service delivery to the Chief Pharmacist and/or Director of Pharmacy.
5. Implement service innovation and improvement initiatives as appropriate to the role. Report and encourage reporting of incidents/near misses to support risk management and quality improvement initiatives.
6. Audit compliance to good clinical practice standards in relation to medication. Compile reports on Key Performance Indicators (KPIs) for the Pharmacy service and disseminate them to stakeholders in a timely manner.
7. Identify drug budget savings in collaboration with Pharmacy and MDT colleagues to ensure optimal and cost-effective medication use.
8. Participate in multidisciplinary meetings, hospital committees, and working groups as required. Co-ordinate service delivery with Pharmacists and Pharmaceutical Technicians to ensure cohesive teamwork within the Pharmacy service.
9. Assist in training, development, mentoring, and motivation of staff to maximize performance and delivery of service objectives and priorities. Ensure adherence to UPMC Human Resource policy and procedures.
10. Engage in continuous professional development appropriate to the role and comply with professional obligations. Have a working knowledge of Health Information and Quality Authority (HIQA) Standards as they apply to the role.
