Job Description
The successful candidate will be responsible for managing patient flow and ensuring the safe and effective management of resident and patient referrals to Leopardstown Care Centre.
To achieve this, the Clinical Nurse Manager (CNM) will work closely with the Patient Flow team to support and deputize for the ADON. The CNM will receive patient referrals from key referral sources, review them, and liaise with the referral teams/discharge coordinators as necessary.
A pre-admission assessment will be completed to ensure that the referral is suitable for placement in Leopardstown Care Centre. For short-term and rehabilitation admissions, the CNM will work with the ADON to set a discharge plan and anticipated date of discharge.
The CNM will also arrange for accepted referrals to be admitted to the centre and act as case manager for patients as required. They will maintain clear, concise, and informative assessments and records for all referrals in accordance with legislation and Mowlam Healthcare standards.
Required Skills and Qualifications
* 3 years post-registration experience in a relevant field and experience in managing patient caseload.
* Hold a recognized managerial qualification (minimum level 6 on the National Framework of Qualifications or be working towards this level of qualification).
* Registered on the NMBI live register.
* Clinical experience working in older persons care or nursing home care.
* Working experience of HIQA Standards and Health Act Regulations 2013.
* Fluent in English language (verbal and written) and eligible to work in Ireland.

This job requires maximum flexibility, and specific duties, tasks, and objectives will be agreed with the post holder periodically by the Director of Nursing in the interests of the service.
