Prominent Duties and Accountabilities:
* Assist patients and their relatives develop strategies to cope with the psychological impact of hospitalization and illness through counseling, support, and information
* Carry out psycho-social assessments where social issues are a factor in illness management and advise multi-disciplinary teams accordingly
* Formulate discharge care plans for patients from the earliest possible time following admission within the context of multidisciplinary collaboration
* Create after-care programs for patients which may include referrals to community-based resources
