With a highly diverse, knowledgeable and inclusive team, a prevailing sense of community signifies our companies culture.We are recognised and accredited with the Great Place to Work certification.Key Duties & Responsibilities:Deliver high quality, safe, effective, person-centred care and services to all residentsEnsure all Family Integration Plans (FIP's) are developed, implemented, updated and evaluated to meet the needs of the individual residents and assisting residents in ensuring that the goals and objectives are met in accordance with their needsEstablish and maintain relationships with residents based on respect and equalitySupport residents to integrate into their new communities – register them with GP's, find schools for children, support them to enrol in english language classes, to obtain their PPSN numbers and medical cardsSupport residents to complete activities of daily living and identify social clubs aligned to their interests e.g. rugby, football, art, dance, drama etc.Empower residents to exercise their autonomy and independence, including proactively supporting their independence through assisting their ongoing education, training and employmentComplete key working duties as assigned by the Social Care LeaderEngage in professional supervision with the Social Care Leader and MDT team as requiredParticipate in the development and implementation of Dídean's Quality and Safety Management System (incident, accident, risk and complaints management, audits, quality initiatives etc.)Maintain a safe environment for residents, staff and visitorsParticipate as a team member of the multi-disciplinary care team in all aspects in the delivery of resident careBe aware of the confidential nature of the work and in particular, the right to resident's confidentiality.
