Position Summary:
The person holding this office shall carry out duties under the management of the Principal Medical Social Worker in relation to the provision and development of social work services.
Clinical caseload is allocated by the Principal Medical Social Worker.
Responsibilities
Principal Duties and Responsibilities
The senior clinical practitioner will be required to demonstrate advanced knowledge of and skills in social work practice.
The senior clinical practitioner will be required to demonstrate best practice through informed decision making and the application of theoretical frameworks in complex and difficult cases.
The senior clinical practitioner will be required to demonstrate leadership skills and will act as a resource within the team regarding new developments in social work practice.
The senior clinical practitioner will be required to be informed of current and pending legislation and social policy, which impact on social work practice.
Area of Responsibility:
The senior clinical practitioner will be required to contribute actively to the development and maintenance of high standards of clinical practice in his/her given area in co-operation and collaboration with Team Leaders and Principal Medical Social Worker.
He/She will also have responsibility for co-ordinating appropriate induction for new members of staff and for coordinating journal clubs and in service training within the medical social work department.
Service Management:
Provide counselling, support, and information to patients and their relatives to assist them in developing strategies to cope with the psychosocial and emotional impact of hospitalisation, and illness.
Comprehensive ongoing work with patients and families, using appropriate intervention models and involving other members of the multi-disciplinary team as appropriate.
Conduct home assessments and follow-up visits when necessary, and participate in pre-discharge home visits with patients.
All work outside the hospital will adhere to the current Medical Social Work policy guidelines.
Carry out psychosocial assessments where social issues are a factor in illness management and advise the inter disciplinary team on assessment outcomes when appropriate.
Formulate discharge care plans for patients when their acute medical care has ended.
The discharge planning process is conducted within the context of the multi-disciplinary team.
Plan after care programmes for patients if appropriate, which may include referral to community based resources and support services.
Participate in the delivery of education and training programmes assigned by the Principal Medical Social Worker.
Be involved in relevant research work and participate in the development of Medical Social Work in the hospital under the direction of the Principal Medical Social Worker.
Keep accurate signed records of all social work interventions in both the Medical file, where appropriate, and the Medical Social Work file.
Ensure that statistical data on service provision is available as requested.
Provide bereavement counselling service for relatives and others affected by the death of a patient of the service.
Perform such other duties as may be assigned from time to time in the hospital and at such other locations as may be required by the Chief Executive Officer or Principal Medical Social Worker.
Qualifications
Selection Criteria:
Selection criteria outline the qualifications, skills, knowledge and/or experience that the successful candidate would need to demonstrate for successful discharge of the responsibilities of the post.
Applications will be assessed on the basis of how well candidates satisfy these criteria.
1. Statutory Registration, Professional Qualifications, Experience, etc
(a) Candidates for appointment must:
(i) Be registered, or be eligible for registration, on the Social Workers Register maintained by the Social Workers Registration Board at CORU.
AND
(ii) Have 3 years full time (or an aggregate of 3 years full time) relevant postqualification experience.AND
(iii) Have the requisite knowledge and ability (including a high standard of suitability and professional ability) for the proper discharge of the duties of the office.
(iv) Provide proof of Statutory Registration on the Social Workers Register maintained by the Social Workers Registration Board at CORU before a contract of employment can be issued.
2. Annual registration
(i) On appointment practitioners must maintain annual registration on the Social Workers Register maintained by the Social Workers Registration Board at CORU.
AND
(ii) Practitioners must confirm annual registration with CORU to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).
3. Health
Candidates for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.
4. Character
Candidates for and any person holding the office must be of good character.
