This dynamic role involves working directly with individuals across the catchment area, helping them participate fully in home and community life. Our Case Management programme is rooted in a biopsychosocial model, delivered through an interdisciplinary team approach that adapts to the resources available locally.The contract available is Specified Purpose (maternity leave), Full-Time, 39 hours.The salary will be based on the HSE 2022+ Grade VII payscale, pro-rated based on hours worked.What You'll Be Doing:Lead case management and care planning for individuals with ABISupport clients in achieving independence and community integrationCo-ordinate with teams an agency to deliver joined-up servicesManage staff rotas, supervision, and service operationsOversee budgets, payroll, and purchasing for the local serviceEnsure compliance with organisational policies and quality standardsPromote community links and access to employment, leisure, and supportsRepresent the organisation and contribute to service developmentWho We're Looking For:We're seeking a compassionate, organised, and proactive professional with experience in case management and community-based rehabilitation. You will be confident working both independently and as part of an interdisciplinary team, with strong leadership skills and a commitment to person-centred care.
