**Job Title: Community Nurse**

**About the Role:**
Community Nurses play a vital role in providing high-quality care and support to people with disabilities, mental health difficulties, and other conditions. As part of our community residential services, you will be working closely with a diverse group of individuals, promoting independence, dignity, and well-being.

**Key Responsibilities:**
The successful candidate will have experience in ID Services (desirable but not essential) and possess excellent communication skills. Full driving license and use of car are required for this role.
- Conduct thorough assessments to identify individualized care plans tailored to each person's unique needs.
- Develop strong relationships with service users, their families/carers/friends
- Collaborate effectively as part of an interdisciplinary team (IDT), including doctors & therapists etc..
- Provide direct care/support where appropriate,
and also facilitate or coordinate when necessary,- Ensure accurate recording/communication about outcomes,effectiveness,& compliance regarding interventions/treatments,equipment/personal aids e.g., nebulisers/artificial limbs dental hygiene/diet/nutrition;< ----------------------------------- - Currently registered with NMBI,
extensive knowledge/experience desirable,basic training mandatory
