Disabilities Case Manager
Medforce are proud to be working with a disability service in Wicklow, offering invaluable services to the local community. This is a full time 35 hour week role Mon to Fri but can offer good flexibility as needed.
Responsibilities of Disabilities Case Manager
* Work as part of the Disability service Team
* Develop relationships and structures with the relevant stakeholders responsible for provision and delivery of services to ensure maximum service provision
* Develop links to the service development process to inform the prioritisation of developments where service gaps have been identified
* As required appraise and develop service options / proposals for individual clients who present as requiring service in conjunction with the client and / or family member / advocate, relevant professionals, agencies and management
* Develop and present a costed business case for provision of services to individual clients as deemed necessary
* Convene, co‑ordinate and / or attend multi /trans disciplinary case management reviews as required
* Develop and promote multidisciplinary, interagency, intersectoral links with relevant statutory, voluntary and private service providers as appropriate
* Deputise for the Disability Manager when required
* Act as Assessment / Liaison Officer (LO) role is set out under Section 11 Disability Act (2005) as service needs require.
* Maintain appropriate, up to date written and electronic records and activity data in accordance with professional service standard and provide reports and data as required
* Ensure that all administrative systems are in accordance with guidelines and to the highest standards
* Efficiently and effectively manage and monitor budgets / expenditure and resources as may be assigned, and to prepare financial management reports as may be required
* Adhere to Policies with regard to procurement of services
* Be responsible for the provision of statistics as requested
* Provide such reports as may be necessary in relation to complaints
* Participate in research and development as appropriate
Requirements of Disabilities Case Manager
* Significant experience of working in Disability Services, as relevant to this role
* Significant experience of Case Management or significant elements of case management processes such as placement, discharge liaison, co‑ordination of packages of care etc, as relevant to this role
* Significant experience working as part of a multi-disciplinary team in Disability Services
