We are currently seeking a dedicated Social Worker to join a dynamic multidisciplinary team, providing essential advocacy and support services to individuals and their families. This role involves managing a caseload of varying complexity while ensuring that each person receives the highest level of care and access to the services they need.
Key Responsibilities:
* Manage a diverse caseload, addressing both individual and family needs.
* Advocate for the rights and entitlements of individuals, ensuring they are informed and supported.
* Facilitate advocacy sessions, guiding individuals through self-advocacy and group advocacy initiatives.
* Identify and address unmet needs, working closely with the team to develop appropriate solutions.
* Participate in care planning and multidisciplinary meetings, collaborating with staff and clinicians to enhance support services.
* Provide safeguarding support, assisting designated officers in protecting vulnerable individuals.
* Contribute to funding applications and reports, ensuring accurate documentation and compliance with best practices.
* Maintain professional standards in line with current legislation and organizational policies.
Requirements:
* Recognized Social Work qualification (NQSW/CQSW) or equivalent.
* CORU registration (or in the process of registering).
* Minimum one year of professional experience in social work.
* Experience working within a multidisciplinary team.
* Strong IT skills for documentation and reporting.
* Full, clean driving license and access to a car.
