Job Title: Healthcare Professional Lead
Overview of the Role
The healthcare team is a collaborative department consisting of experienced professionals, including a lead healthcare professional, staff nurses and administrative support. They provide high-quality patient care across multiple disciplines.
The Lead Healthcare Professional will oversee patient care to ensure the highest professional standards using evidence-based practices. They will be responsible for coordinating, assessing, planning, delivering and reviewing patient care by all staff in designated areas. The role includes providing safe, comprehensive nursing care to patients according to professional codes and guidelines.
Key Responsibilities
* Manage own caseload in accordance with patient needs
* Participate in teams / meetings as appropriate, communicating and working collaboratively with other team members
* Facilitate coordination, cooperation and liaison across healthcare teams
* Collaborate with patients, families, carers and other staff in treatment and care planning
* Communicate verbally and/or in writing results of assessments, treatment plans and recommendations to the team and relevant others
* Plan discharge or transition of patients between services as necessary
* Ensure that patients and others are treated with dignity and respect
* Maintain accurate and up-to-date patient records in accordance with local and professional standards
Requirements
* Professional Qualifications, Experience:
o Eligible applicants must be registered nurses with at least 5 years post-registration experience, including 2 years in the specialty or related area
o Candidates must demonstrate evidence of continuous professional development
* Annual Registration:
o On appointment, practitioners must maintain live annual registration on the register of nurses maintained by the Nursing and Midwifery Board Ireland (NMBI)
o Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC)
