Case Manager Opportunity
We are seeking a qualified and dedicated Case Manager to join our interdisciplinary team supporting children with complex needs.
The successful candidate will conduct comprehensive case assessments, provide family support and counseling, and collaborate with other professionals to ensure high-quality care.
* Conduct assessments of children and families with developmental and disability-related needs, including gathering information from various sources such as healthcare providers, teachers, and social services
* Provide family support, psychoeducation, and practical guidance to help families navigate the complexities of their child's condition
* Develop and review Individual Family Service Plans (IFSPs) in collaboration with the multidisciplinary team
* Work collaboratively within a multidisciplinary team to ensure a holistic approach to care
* Assist families in navigating disability services and advocacy pathways, connecting them with relevant resources and support
Key Responsibilities:
Assessment and Planning:
Conduct comprehensive assessments of children and families with developmental and disability-related needs, including gathering information from various sources such as healthcare providers, teachers, and social services
Develop and review Individual Family Service Plans (IFSPs) in collaboration with the multidisciplinary team
Family Support and Counseling:
Provide family support, psychoeducation, and practical guidance to help families navigate the complexities of their child's condition
Assist families in developing coping strategies and providing emotional support during challenging times
Collaboration and Communication:
Work collaboratively within a multidisciplinary team to ensure a holistic approach to care
Communicate effectively with families, healthcare providers, and other stakeholders to ensure seamless coordination of care
Advocacy and Resource Navigation:
Assist families in navigating disability services and advocacy pathways, connecting them with relevant resources and support
Facilitate access to community resources, government benefits, and other services that support the well-being of children and families
Professional Development:
Pursue ongoing professional development opportunities to stay current on best practices in case management and related fields
Participate in regular supervision and peer consultation to enhance practice skills and address challenges
