Key Worker Role
The purpose of this role is to provide a Key Work service that seeks to improve the health and wellbeing of our clients within their local communities.
Key Responsibilities:
* Develop, plan, implement and review individualised support/intervention programmes for clients according to service standards.
* Ensure best practice in line with up-to-date research regarding person-centred support/intervention in line with New Directions.
* Increase awareness, education and understanding of Asperger Syndrome/ASD Non-ID among professionals and the wider community.
* Liaise with management regarding referrals and transitions between different elements of the service as appropriate.
* Collaborate with clients and document all client engagement, intervention, support plans, progress notes, reports and referrals to agreed professional standards.
Required Skills and Qualifications:
* Ability to develop and maintain effective relationships with clients, colleagues and other stakeholders.
* Strong communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Knowledge of relevant legislation and policies affecting vulnerable adults.
* Understanding of the principles of safeguarding and protection from abuse.
