Key Responsibilities:
* Support individuals to achieve their goals and aspirations, promoting autonomy and self-reliance.
* Collaborate with service users, families, and other stakeholders to develop person-centered plans that meet individual needs.
* Establish and maintain effective relationships with local communities, fostering active citizenship and participation.
* Provide safe and reliable transportation for service users to access community resources.
* Manage key worker duties, including regular administration, advocacy, and liaison with families.
Requirements:
* Hold a relevant qualification - minimum Level 5 in Healthcare or equivalent.
* Experience working with vulnerable populations, particularly those with disabilities.
* Be over 18 years old and possess a full clean manual driving license with access to own vehicle for work purposes.
* Have excellent communication and interpersonal skills, with the ability to engage effectively with diverse groups.
* Previous experience working with individuals with disabilities or in a similar sector is highly desirable.
Hours: Monday to Friday, with some Saturday shifts as required. Based in Kilkenny, with opportunities for professional growth and development.
