Job Description:

Responsibilities include:
* Motivating and supporting service users in engaging with their preferred activities.
* Supporting personal care and daily needs of service users.
* Promoting independence and community integration in accordance with New Directions (HSE, 2012).
* Educating oneself about each service user, listening actively, and using reflective practice for continuous improvement.
* Participating as a key worker with guidance from senior staff.
* Working collaboratively within a team, supporting colleagues, attending meetings, and engaging in supervision.
* Maintaining confidentiality in line with GDPR.
* Managing medication as per policy.
* Liaising with multidisciplinary teams, families, and other stakeholders.
* Carrying out health and safety observations and reporting.
* Contributing to person-centred planning and follow-up actions.
* Supporting service users in daily life and decision-making.
* Reporting concerns regarding welfare, safety, and health.
* Accurately recording information according to standards.
* Supporting behavior management using individualized positive behavior support plans.
* Ensuring quality standards through audits and follow-up actions.
* Following financial procedures and reporting discrepancies.
* Building positive relationships with families and stakeholders.
* Developing professional rapport with colleagues to ensure effective service delivery.
* Creating community participation opportunities for service users.
* Supporting volunteer, employment, and educational pursuits.
* Attending mandatory and developmental training programs.
* Engaging in supervision and appraisal, reflecting on practice for ongoing development.
* Performing any other duties relevant to the social care worker role to meet service needs.
Qualifications required:
* Minimum Level 7 in Applied Social Care or a relevant qualification.
* Full driver's license with at least two years of experience.
* Currently undertaking or holding a CORU-recognized Social Care Qualification.
