Responsibilities
* Motivate, support, and encourage service users to engage in planned activities of their choice.
* Support the care of service users, including personal care and providing a general overview of their daily needs.
* Support and promote independence and community integration in line with New Directions (HSE, 2012).
* Educate yourself about each service user, listen to them, and use reflective practice to develop your skills and knowledge.
* Participate in a key worker role under guidance from senior staff.
* Work as part of a team, supporting colleagues and attending regular meetings and supervisions.
* Uphold confidentiality in accordance with GDPR.
* Manage and administer medication following policy.
* Liaise with multidisciplinary teams, families, and relevant parties regarding service users.
* Observe and report on health and safety matters.
* Contribute to Person-Centred Planning meetings and follow up on actions and outcomes.
* Ensure service users receive support in daily life and make appropriate choices.
* Report concerns related to welfare, safety, and health.
* Record accurately following National Standards and New Directions.
* Support behavior management using positive behavior support plans tailored to each individual.
* Maintain high quality standards through audits and follow-up actions.
* Follow financial procedures, report discrepancies.
* Build positive relationships with families, colleagues, and the community.
* Create opportunities for community participation, volunteering, employment, and education.
* Attend mandatory training and development programs.
* Engage in supervision and appraisals, using reflective practice for ongoing development.
* Perform any other duties within the scope of the Support Worker role to meet service needs.
Qualifications
* Relevant healthcare qualification (minimum Fetac Level 5 or equivalent in Social Care).
* Full driver’s license with at least two years of experience.
* Currently undertaking or holding a CORU-recognized Social Care Qualification.
