Requirements:
* Master's degree in nursing or Physician Assistant Studies
* Valid California License as a Registered Nurse or Physician Assistant
* Certification from the State of California Board of Registered Nursing or the Physician Assistant Board
* National Provider Identification (NPI) number and DEA Registration
