Lead Dementia Care Specialist
As a key member of our specialist team, the Clinical Lead Nurse in Dementia Care will provide comprehensive physical, psychological, and emotional assessment and support to people with dementia and their families/carers throughout their disease trajectory.
This role involves working collaboratively with multidisciplinary teams to promote integrated care pathways across acute and community services. The successful candidate will have an operational clinical working relationship with Geriatricians leading on Dementia Services within the hospital.
The purpose of this role is to deliver a specialist nursing service for patients with dementia that requires support and treatment through the continuum of care. This includes conducting early identification and assessment of patients with cognitive impairment, participating in case review, and taking a proactive role in policy formulation and provision related to dementia care.
